<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\" target=\"_blank\">Lee S, Kim, et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2001737\" data-type=\"URL\" data-id=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2001737\" target=\"_blank\">Zou L, Ruan, et al.<\/a>).<\/p>\n\n\n\n<p>But symptomatic cases transmitted to more people than asymptomatic cases, according to surveys of Wuhan households (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30981-6\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30981-6\" target=\"_blank\">Li F, Li Y-Y, et al.<\/a>), Luxembourg (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100056\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100056\" target=\"_blank\">Wilmes et al.<\/a>), and Japanese clusters (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.ijid.2021.02.065\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.ijid.2021.02.065\" target=\"_blank\">Nakajo, Nishiura \u201cTransmissibility\u201d<\/a>), systematic reviews and meta-analyses of many studies (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2774102\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2774102\" target=\"_blank\">Madewell et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.imperial.ac.uk\/mrc-global-infectious-disease-analysis\/covid-19\/report-38-transmission\/\" data-type=\"URL\" data-id=\"https:\/\/www.imperial.ac.uk\/mrc-global-infectious-disease-analysis\/covid-19\/report-38-transmission\/\" target=\"_blank\">Thompson H, Mousa, et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.imperial.ac.uk\/news\/209673\/covid-19-spread-different-social-settings-imperial\/\" data-type=\"URL\" data-id=\"https:\/\/www.imperial.ac.uk\/news\/209673\/covid-19-spread-different-social-settings-imperial\/\" target=\"_blank\">van Elsland<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/apps.who.int\/iris\/rest\/bitstreams\/1279750\/retrieve\" data-type=\"URL\" data-id=\"https:\/\/apps.who.int\/iris\/rest\/bitstreams\/1279750\/retrieve\" target=\"_blank\">World Health Organization \u201cAdvice on the Use of Masks\u201d<\/a>), and contact tracing studies (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32651-9\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32651-9\/fulltext\" target=\"_blank\">Sayamphanathan et al.<\/a>). &nbsp;For example, in two studies in China, symptomatic cases were 4 times and 10 times more likely to transmit than asymptomatic cases (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" target=\"_blank\">Ge et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3389\/fmed.2021.591372\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3389\/fmed.2021.591372\" target=\"_blank\">Li C, Zhu Y, Zhong Y, et al.<\/a>).&nbsp; During the early months of the pandemic in the US, symptomatic patients infected 29% of their household contacts (secondary infection rate, SIR), including 42% of their children and 33% of spouses\/ partners (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1093\/cid\/ciaa1166\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/cid\/ciaa1166\" target=\"_blank\">Lewis NM, Chu VT, et al.<\/a>). <\/p>\n\n\n\n<p>But a study of the Omicron variant in households in Shanghai, China found about equal infectivity in asymptomatic and symptomatic cases. Many got infected despite a city-wide lockdown and centralized isolation and quarantine (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1111\/irv.13097\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1111\/irv.13097\" target=\"_blank\">Wei Z, Ma W, et al.<\/a>).<\/p>\n\n\n\n<p>Several studies found large percentages of people had asymptomatic infections, varying with their ages and transmission situations (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M20-3012\" data-type=\"URL\" data-id=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M20-3012\" target=\"_blank\">Oran, Topol<\/a>).&nbsp; For example, a longitudinal serological survey of the first epicenter, Wuhan, China, showed that 82% of those with antibodies had been asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00238-5\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00238-5\" target=\"_blank\">He Z, Ren, et al<\/a>.).&nbsp; In Taipei City in 2021, 42% &#8211; 46% of SARS-CoV-2 infected hospital inpatients and health care workers were asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1155\/2022\/6441339\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1155\/2022\/6441339\" target=\"_blank\">Cheng CC et al. \u201cSystematic Screening\u201d<\/a>).&nbsp; A survey of half the Luxembourg resident population found 33% of positive cases were asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100056\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100056\" target=\"_blank\">Wilmes et al.<\/a>).&nbsp; In the heavily infected Italian town of Vo, testing most of the population twice found about 40% of infected people had no symptoms, and the symptomatic and asymptomatic cases had about equal viral loads that could transmit to others (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.eurekalert.org\/pub_releases\/2020-06\/icl-wsr063020.php\" data-type=\"URL\" data-id=\"https:\/\/www.eurekalert.org\/pub_releases\/2020-06\/icl-wsr063020.php\" target=\"_blank\">Imperial College London<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nature.com\/articles\/s41586-020-2488-1\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41586-020-2488-1\" target=\"_blank\">Lavezzo et al.<\/a>).&nbsp; In the general population of a high transmission area, among pregnant women admitted to a New York City hospital for birth delivery, 14% had SARS-CoV-2 without symptoms (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2009316\" data-type=\"URL\" data-id=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2009316\" target=\"_blank\">Sutton et al.<\/a>).&nbsp; Among people found infected in a program in Korea, 36% were asymptomatic.&nbsp; Then 19% started having symptoms, showing that they had actually been presymptomatic (<a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\">Lee S, Kim, et al.<\/a>). &nbsp; After several residents of a long-term care skilled nursing facility in Washington state had COVID-19, scientists tested 93% of residents, and found that about half of those infected did not show symptoms (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6913e1.htm\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6913e1.htm\" target=\"_blank\">Kimball et al.<\/a>). &nbsp;When large numbers of cruise ship passengers and crews were tested for viral RNA, 26% &#8211; 51% of those infected were asymptomatic. &nbsp;When they tested the same individuals repeatedly, 17.9% of those infected never showed symptoms (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.eurosurveillance.org\/content\/10.2807\/1560-7917.ES.2020.25.10.2000180\" data-type=\"URL\" data-id=\"https:\/\/www.eurosurveillance.org\/content\/10.2807\/1560-7917.ES.2020.25.10.2000180\" target=\"_blank\">Mizumoto et al.<\/a>). &nbsp;So, much of the transmission on the cruise ships was probably from asymptomatic people (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6912e3.htm\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6912e3.htm\" target=\"_blank\">Moriarty et al.<\/a>). &nbsp;Many children with few or no symptoms shed large numbers of coronaviruses, and so could be transmitting it to adults, who could later develop severe infections (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30236-X\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30236-X\/fulltext\" target=\"_blank\">Kelvin and Halperin<\/a>; <a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30198-5\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30198-5\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Qiu et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nature.com\/articles\/s41591-020-0817-4\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41591-020-0817-4\" target=\"_blank\">Xu Y, Li X et al.<\/a>).&nbsp; In London care homes with no reported COVID-19 cases, an antibody seropositivity survey found they ranged from 10% &#8211; 56%, revealing silent transmission (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100038\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.lanepe.2021.100038\" target=\"_blank\">Jeffery-Smith et al.<\/a>).&nbsp; In Henan China early in the pandemic about 42% of infected people were asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3389\/fmed.2021.591372\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3389\/fmed.2021.591372\" target=\"_blank\">Li C, Zhu Y, et al.<\/a>). &nbsp;A survey of University of Arizona students found 79.2% of infected were asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.scitotenv.2021.149794\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.scitotenv.2021.149794\" target=\"_blank\">Schmitz et al.<\/a>).&nbsp; A review of initial epidemics in several countries found 5% &#8211; 80% of cases were asymptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cebm.net\/covid-19\/covid-19-what-proportion-are-asymptomatic\/\" data-type=\"URL\" data-id=\"https:\/\/www.cebm.net\/covid-19\/covid-19-what-proportion-are-asymptomatic\/\" target=\"_blank\">Heneghan, Brassey, et al.<\/a>). 0.5% of the population had asymptomatic infection in a US Air Force base (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3389\/fmed.2021.749732\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3389\/fmed.2021.749732\" target=\"_blank\">Breedon et al.<\/a>).<\/p>\n\n\n\n<p>COVID-19 can spread more easily than many other infectious respiratory diseases, including in the incubation period and asymptomatic infections (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" data-type=\"URL\" data-id=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" target=\"_blank\">Christakis<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" target=\"_blank\">Ge et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nejm.org\/doi\/pdf\/10.1056\/NEJMe2009758\" data-type=\"URL\" data-id=\"https:\/\/www.nejm.org\/doi\/pdf\/10.1056\/NEJMe2009758\" target=\"_blank\">Gandhi M, Yokoe, et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.statnews.com\/2020\/12\/14\/portrait-of-the-coronavirus-at-1\/\" data-type=\"URL\" data-id=\"https:\/\/www.statnews.com\/2020\/12\/14\/portrait-of-the-coronavirus-at-1\/\" target=\"_blank\">Joseph<\/a>), or within a few days after symptom onset (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2765641\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2765641\" target=\"_blank\">Cheng HY, Jian SW \u201cContact Tracing\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.4686\" target=\"_blank\">Ge et al.<\/a>).&nbsp; Symptoms usually start about 5 days after exposure, ranging from 2 -14 days.&nbsp; This virus quickly multiplies in the person, and produces millions of viruses in mucous and saliva in a few days (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1038\/s41591-021-01296-8\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1038\/s41591-021-01296-8\" target=\"_blank\">Huang N, P\u00e9rez, et al.<\/a>).&nbsp; Infected people can shed viable viruses 6 days before symptom onset.&nbsp; After symptoms stop, viable viruses still shed from that person for up to 20 days, and viral RNA for up to 92 days, with a median of 18.4 days (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1017\/ice.2020.1273\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1017\/ice.2020.1273\" target=\"_blank\">Fontana et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1007\/s42399-020-00499-3\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1007\/s42399-020-00499-3\" target=\"_blank\">Karia et al.<\/a>).&nbsp; Asymptomatic and symptomatic cases had similar viral loads and cell culture infectivity the first few days.&nbsp; But most asymptomatic cases reduced viral loads by the third day after diagnosis.&nbsp; Among symptomatic cases, 4% were still infectious 8 days after symptoms started (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3390\/v13112133\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3390\/v13112133\" target=\"_blank\">Bae S, Kim JY, et al.<\/a>).&nbsp; Patients start to develop immune antibodies in a few days after exposure, but the viruses continue multiplying and shedding, and gradually decrease later (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.acpjournals.org\/doi\/full\/10.7326\/M20-0504\" data-type=\"URL\" data-id=\"https:\/\/www.acpjournals.org\/doi\/full\/10.7326\/M20-0504\" target=\"_blank\">Lauer et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"http:\/\/nap.edu\/25774\" data-type=\"URL\" data-id=\"http:\/\/nap.edu\/25774\" target=\"_blank\">National Academies of Sciences, &#8220;SARS-CoV-2 Viral shedding&#8221;<\/a>; In that sequence of events, cases are more likely to transmit when presymptomatic than symptomatic (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30981-6\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30981-6\" target=\"_blank\">Li F, Li Y-Y, et al.<\/a>).&nbsp; These processes facilitate people spreading SARS-CoV-2 through their social networks more than most infectious diseases (<a href=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" data-type=\"URL\" data-id=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" target=\"_blank\" rel=\"noreferrer noopener\">Christakis<\/a>).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Isolation and quarantine<\/h2>\n\n\n\n<p>[Publication date of latest article cited: October 19, 2023]<\/p>\n\n\n\n<p>Since many infected people are asymptomatic or presymptomatic, as described above, they should isolate themselves from others (<a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769235\" target=\"_blank\" rel=\"noreferrer noopener\">Lee S, Kim, et al.<\/a>).&nbsp; Altogether, people with COVID-19 symptoms can probably transmit to others about two days before symptoms begin, and then for about another 7-10 days.&nbsp; After symptoms start, SARS-Cov-2 replication probably continues for about a week in most patients (<a href=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(20)30172-5\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(20)30172-5\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Cevik, Tate, et al.<\/a>), then usually decreases so viruses could not be cultured from most patients after two weeks.&nbsp; But weeks later some patients might still have non-infectious RNA found by PCR (<a href=\"https:\/\/doi.org\/10.1016\/j.jcv.2021.105060\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.jcv.2021.105060\" target=\"_blank\" rel=\"noreferrer noopener\">Anantharaj et al.<\/a>; <a href=\"https:\/\/www.ams.edu.sg\/view-pdf.aspx?file=media%5c5558_fi_168.pdf&amp;ofile=Period+of+Infectivity+Position+Statement+(final)+23-5-20.pdf\" data-type=\"URL\" data-id=\"https:\/\/www.ams.edu.sg\/view-pdf.aspx?file=media%5c5558_fi_168.pdf&amp;ofile=Period+of+Infectivity+Position+Statement+(final)+23-5-20.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">National Centre for Infectious Diseases<\/a>).<\/p>\n\n\n\n<p>In the first year of the pandemic, US CDC experts recommended that people who have been infected should isolate from other people for at least 10 days after symptoms started, and their symptoms improved, and they have not had a fever for 24 hours without using fever reducing medicines.&nbsp; After meeting all those conditions, they can come out from isolation &nbsp; (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/duration-isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/duration-isolation.html\" target=\"_blank\">Centers for Disease Control \u201cDuration of Isolation\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/community\/strategy-discontinue-isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/community\/strategy-discontinue-isolation.html\" target=\"_blank\">Centers for Disease Control \u201cSymptom based strategy\u201d<\/a>; After two pandemic years, research showed that infected people transmit more in the two days before symptoms, and three days after starting symptoms, than in the days after that.&nbsp; So, by late 2021 experts recommended that infected people isolate for five days, and longer if they have symptoms, especially fever.&nbsp; Then, if they have no fever, they should wear a mask when near people for five more days (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/media\/releases\/2021\/s1227-isolation-quarantine-guidance.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/media\/releases\/2021\/s1227-isolation-quarantine-guidance.html\" target=\"_blank\">Centers for Disease Control \u201cCDC Updates and Shortens\u2026\u201d<\/a>).  &nbsp; By late 2022, they recommended isolating for five days, up to ten if one had moderate symptoms, and avoiding people \u201cmore likely to get very sick.\u201d &nbsp;One should get tested, and if the test is positive, one should isolate until two tests show negative (<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/isolation.html\" target=\"_blank\" rel=\"noreferrer noopener\">Centers for Disease Control \u201cIsolation and Precautions\u201d<\/a>; <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/isolation.html\" target=\"_blank\" rel=\"noreferrer noopener\">Centers for Disease Control \u201cIsolate and Take Precautions\u201d<\/a>).&nbsp; &nbsp;<\/p>\n\n\n\n<p>People who have been exposed to an infected person, but do not show symptoms, should prevent transmitting to others.&nbsp; Called &#8220;contacts,&#8221; they should remain quarantined, avoid contaminating surfaces or exhaling in rooms used by others. &nbsp;In the first year of the pandemic, before vaccines were available, US CDC experts recommended that exposed contacts should quarantine for 10 days if they do not develop symptoms, or for 7 days if they test negative.&nbsp; Or they could remain quarantined for 14 days if necessary (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/if-you-are-sick\/quarantine.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/if-you-are-sick\/quarantine.html\" target=\"_blank\">Centers for Disease Control \u201cWhen to Quarantine\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/community\/strategy-discontinue-isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/community\/strategy-discontinue-isolation.html\" target=\"_blank\">Centers for Disease Control \u201cSymptom based strategy\u201d<\/a>; After two pandemic years, when millions had been vaccinated, in late 2021 experts recommended that if people have had three vaccine doses, and then were exposed, they should wear a mask near others for ten days, but do not need to quarantine.&nbsp; People who were unvaccinated or incompletely vaccinated, and then exposed, should quarantine for five days, and then wear a mask near others for five days. &nbsp;If they do not quarantine, they should wear a correctly fitting mask for ten days (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/media\/releases\/2021\/s1227-isolation-quarantine-guidance.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/media\/releases\/2021\/s1227-isolation-quarantine-guidance.html\" target=\"_blank\">Centers for Disease Control \u201cCDC Updates and Shortens\u2026\u201d<\/a>). By 2022, experts made similar recommendations, including testing after the fifth day. If the test is negative, continue taking precautions until the tenth day. If the test is positive, then isolate according to recommendations in the paragraph above (<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/if-you-were-exposed.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/your-health\/if-you-were-exposed.html\" target=\"_blank\" rel=\"noreferrer noopener\">Centers for Disease Control \u201cWhat to Do\u201d<\/a>).<\/p>\n\n\n\n<p>People who live with COVID-19 patients should protect themselves for weeks afterwards. &nbsp; When scientists followed up both ordinary COVID-19 patients (<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2762028\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2762028\" target=\"_blank\" rel=\"noreferrer noopener\">Bai et al.<\/a>; <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s11427-020-1661-4\" data-type=\"URL\" data-id=\"https:\/\/link.springer.com\/article\/10.1007\/s11427-020-1661-4\" target=\"_blank\" rel=\"noreferrer noopener\">Hu et al.<\/a>; <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1684118220300815b\" data-type=\"URL\" data-id=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1684118220300815b\" target=\"_blank\" rel=\"noreferrer noopener\">Xing et al.<\/a>) and medical staff patients (<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2762452\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2762452\" target=\"_blank\" rel=\"noreferrer noopener\">Lan et al.<\/a>), they found that most of those recovered had no SARS-CoV-2 RNA, then later started having viral RNA again. &nbsp;Their families living with them used infection prevention methods, but some of their family members were infected.<\/p>\n\n\n\n<p>These patterns of symptoms, RNA, and viruses could allow many asymptomatic and presymptomatic people to continue transmitting to many others. &nbsp;This could&nbsp; permit the pandemic to continue for many months, and resurge in communities that relax social distancing, testing, and treatment (<a href=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" data-type=\"URL\" data-id=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" target=\"_blank\" rel=\"noreferrer noopener\">Christakis<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/academic.oup.com\/cid\/advance-article\/doi\/10.1093\/cid\/ciaa1313\/5900759\" data-type=\"URL\" data-id=\"https:\/\/academic.oup.com\/cid\/advance-article\/doi\/10.1093\/cid\/ciaa1313\/5900759\" target=\"_blank\">Clipman et al.<\/a>).<\/p>\n\n\n\n<p>Contact tracing, isolation, and quarantining succeeded in reducing transmission.&nbsp; For example, in some fitness centers in Korea, contact tracing and social distancing stopped an outbreak (<a rel=\"noreferrer noopener\" href=\"https:\/\/jkms.org\/DOIx.php?id=10.3346\/jkms.2020.35.e288\" data-type=\"URL\" data-id=\"https:\/\/jkms.org\/DOIx.php?id=10.3346\/jkms.2020.35.e288\" target=\"_blank\">Bae, Kim, et al.<\/a>). &nbsp;In a US university, these methods prevented almost all widespread transmission, and stopped the two outbreaks (<a href=\"https:\/\/doi.org\/10.1093\/infdis\/jiad397\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1093\/infdis\/jiad397\" target=\"_blank\" rel=\"noreferrer noopener\">Turcinovic et al.<\/a>). &nbsp;In a French hospital program in an area where many people practiced quarantining and other non-pharmaceutical interventions, during this pandemic influenza and respiratory syncytial virus cases were reduced to fractions of those in past years.&nbsp; Rhinovirus cases decreased among adults, and increased among children, possibly because schools remained open and children were not required to wear masks (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1002\/jmv.26959\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1002\/jmv.26959\" target=\"_blank\">Mansuy et al.<\/a>).&nbsp; But large minorities of infected people and contacts did not regularly do the recommended behaviors because they did not access community support and tests, lacked control over leaving home, did not perceive benefit in following the rules, felt uncertain about their symptoms, and distrusted the Government (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3390\/ijerph18137015\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3390\/ijerph18137015\" target=\"_blank\">Eraso et al.<\/a>).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Immunity<\/h2>\n\n\n\n<p>[Publication date of latest article cited: June 22, 2023]<\/p>\n\n\n\n<p>After people have COVID-19, most become partially immune.  They can get COVID-19 several times, but most have less symptoms in later infections, and some have more symptoms, and some did not get infected again (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" data-type=\"URL\" data-id=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" target=\"_blank\">Kim DS, Roland-Jones ,et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2034545\" data-type=\"URL\" data-id=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2034545\" target=\"_blank\">Lumley et al. \u201cAntibody Status and Incidence\u201d<\/a>). &nbsp;For example, a study of 69% of the Danish population found that among those not infected in the first surge, 3.27% were infected in the second surge.&nbsp; In comparison, among those infected in the first surge, only 0.65% were infected in the second surge.&nbsp; This means that natural infection provides 80% protection for those aged under 65, and 47% for 65 and older (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00662-0\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00662-0\" target=\"_blank\">Boyton, Altmann<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00575\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00575\" target=\"_blank\">Hansen et al.<\/a>). Infection provided protective effectiveness of 74.7% against severe infection or hospitalization at 12 months, and 24.3% against reinfection at 12 months (<a href=\"https:\/\/doi.org\/10.1016\/S1473-3099(22)00801-5\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1016\/S1473-3099(22)00801-5\" target=\"_blank\" rel=\"noreferrer noopener\">Bobrovitz et al.<\/a>).<\/p>\n\n\n\n<p>The body\u2019s humoral immune system responds soon, creating antibodies to antigen molecules on SARS-CoV-2. Many convalescent patients developed SARS-CoV-2 neutralizing antibodies, IgM, IgG, and IgA which decreased months later ( <a rel=\"noreferrer noopener\" href=\"https:\/\/www.mdpi.com\/1422-0067\/21\/13\/4782\" data-type=\"URL\" data-id=\"https:\/\/www.mdpi.com\/1422-0067\/21\/13\/4782\" target=\"_blank\">Allegra et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1080\/21505594.2021.1929800\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1080\/21505594.2021.1929800\" target=\"_blank\">Alshamarry et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/eji.202048970\" data-type=\"URL\" data-id=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/eji.202048970\" target=\"_blank\">Figueiredo\u2010Campos et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/link.springer.com\/article\/10.1007\/s12250-020-00329-9\" data-type=\"URL\" data-id=\"https:\/\/link.springer.com\/article\/10.1007\/s12250-020-00329-9\" target=\"_blank\">Zhang X, Lu, et al.<\/a>), and 69%-91% had antibodies after 8 months (<a rel=\"noreferrer noopener\" href=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/27\/3\/20-4543_article\" data-type=\"URL\" data-id=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/27\/3\/20-4543_article\" target=\"_blank\">Choe P, Kim, et al.<\/a>).&nbsp; A longitudinal serological survey in Wuhan found that, among people with antibodies, about 40% had neutralizing antibodies during the nine months studied (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00434-7\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00434-7\" target=\"_blank\">He Z, Ren, et al.<\/a>).&nbsp; As their antibodies increased, the number of viruses they had (called \u201cviral load\u201d) decreased (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.jcv.2021.105060\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.jcv.2021.105060\" target=\"_blank\">Anantharaj et al.<\/a>). Many adolescents infected with the original 2020 wild type virus developed persisting antibodies to the wild type, but lower antibody titers against the later Alpha, Beta, Delta, and Omicron variants (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1097\/inf.0000000000003890\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1097\/inf.0000000000003890\" target=\"_blank\">Aiano et al.<\/a>).<\/p>\n\n\n\n<p>Having antibodies probably protected people from infection (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" data-type=\"URL\" data-id=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" target=\"_blank\">Kim et al.<\/a>).&nbsp; For a small scale example, on a fishing boat, three crew members had antibodies.&nbsp; During their next voyage, one other person infected most of the others, but not the three already having antibodies (<a rel=\"noreferrer noopener\" href=\"https:\/\/jcm.asm.org\/content\/early\/2020\/08\/21\/JCM.02107-20\" data-type=\"URL\" data-id=\"https:\/\/jcm.asm.org\/content\/early\/2020\/08\/21\/JCM.02107-20\" target=\"_blank\">Addetia et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nytimes.com\/2020\/10\/20\/magazine\/covid-natural-experiments.html\" data-type=\"URL\" data-id=\"https:\/\/www.nytimes.com\/2020\/10\/20\/magazine\/covid-natural-experiments.html\" target=\"_blank\">Tingley<\/a>).&nbsp; For a larger scale example, one study showed that among healthcare workers with antibodies, none developed symptomatic infection in the next 6 months (<a href=\"http:\/\/nejm.org\/doi\/full\/10.1056\/NEJMoa2034545\" data-type=\"URL\" data-id=\"http:\/\/nejm.org\/doi\/full\/10.1056\/NEJMoa2034545\" target=\"_blank\" rel=\"noreferrer noopener\">Lumley et al. \u201cAntibody Status and Incidence\u201d<\/a>).&nbsp; A study of patients for 6 \u2013 8 months found spike IgG antibodies were stable, spike-specific memory B cells increased from 1 to 6 months, and CD4<sup>+<\/sup> T cells and CD8<sup>+<\/sup> declined (<a href=\"https:\/\/doi.org\/10.1126\/science.abf4063\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1126\/science.abf4063\" target=\"_blank\" rel=\"noreferrer noopener\">Dan et al.<\/a>).&nbsp; A nation-wide follow-up of millions found that those initially having antibodies later had declining ratios of diagnostic nucleic acid amplification tests (NAAT) relative to people who initially lacked antibodies.&nbsp; This indirectly suggests that people with antibodies had SARS-CoV-2 at first, which disappeared, and then they did not get infected again (<a href=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.0366\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jamainternmed.2021.0366\" target=\"_blank\" rel=\"noreferrer noopener\">Harvey R, Rassen,et al.<\/a>).&nbsp; Individuals\u2019 immune responses varied greatly, with different amounts of IgM, IgG, IgA, and neutralizing antibodies (Nab) resulting in differing immunity (<a href=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" data-type=\"URL\" data-id=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" target=\"_blank\" rel=\"noreferrer noopener\">Kim DS, Roland-Jones,&nbsp; et al.<\/a>).<\/p>\n\n\n\n<p>Then the body\u2019s cellular immune system responds, creating memory T cells, CD4+ helper T cells, CD8+ killer T cells, and memory B cells, which are kinds of white cell lymphocytes.&nbsp; They develop in weeks, are detectable for 6 \u2013 8 months, and provide immunity for over 6 months (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.mdpi.com\/1422-0067\/21\/13\/4782\" data-type=\"URL\" data-id=\"https:\/\/www.mdpi.com\/1422-0067\/21\/13\/4782\" target=\"_blank\">Allegra et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1080\/21505594.2021.1929800\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1080\/21505594.2021.1929800\" target=\"_blank\">Alshamarry et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1038\/s41590-021-00902-8\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1038\/s41590-021-00902-8\" target=\"_blank\">Zuo J et al.<\/a>). &nbsp;Individuals\u2019 immune responses varied greatly, with different counts of memory T cells, CD4+ T cells, CD8+ T cells, and memory B cells associated with in differing immunity (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" data-type=\"URL\" data-id=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" target=\"_blank\">Kim DS, Roland-Jones, et al.<\/a>).<\/p>\n\n\n\n<p>Some people who probably had previous coronaviruses, such as SARS, MERS, or common colds, developed T cells which could attack SARS-CoV-2 (<a href=\"https:\/\/www.nature.com\/articles\/s41586-020-2550-z\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41586-020-2550-z\" target=\"_blank\" rel=\"noreferrer noopener\">Le Bert et al.<\/a>).&nbsp; Consequently, patients with no detectable antibodies might still be immune, because the T cells, B cells, and small amount of antibodies could prevent infection (<a href=\"https:\/\/www.nytimes.com\/2020\/07\/31\/opinion\/coronavirus-antibodies-immunity.html\" data-type=\"URL\" data-id=\"https:\/\/www.nytimes.com\/2020\/07\/31\/opinion\/coronavirus-antibodies-immunity.html\" target=\"_blank\" rel=\"noreferrer noopener\">Iwasaki, Medzhitov<\/a>; <a href=\"https:\/\/www.nature.com\/articles\/s41577-020-0389-z\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41577-020-0389-z\" target=\"_blank\" rel=\"noreferrer noopener\">Sette, Crotty<\/a>).&nbsp; Altogether this shows why vaccines effectively immunize many people (<a href=\"https:\/\/blogs.sciencemag.org\/pipeline\/archives\/2020\/08\/18\/encouraging-news-about-coronavirus-immunity\" data-type=\"URL\" data-id=\"https:\/\/blogs.sciencemag.org\/pipeline\/archives\/2020\/08\/18\/encouraging-news-about-coronavirus-immunity\" target=\"_blank\" rel=\"noreferrer noopener\">Lowe<\/a>).&nbsp; Patients\u2019 antibodies, T cells, and B cells varied, showing why disease-induced, and vaccine-induced immunity among large populations varies (<a href=\"https:\/\/www.nature.com\/articles\/s41591-020-0995-0\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41591-020-0995-0\" target=\"_blank\" rel=\"noreferrer noopener\">Juno et al.<\/a>; <a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.03.30.20047365v2\" data-type=\"URL\" data-id=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.03.30.20047365v2\" target=\"_blank\" rel=\"noreferrer noopener\">Wu F, Liu, et al.<\/a>).&nbsp; But scientists do not yet know how long immunity will last (<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/duration-isolation.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/duration-isolation.html\" target=\"_blank\" rel=\"noreferrer noopener\">Centers for Disease Control \u201cDuration of Isolation\u201d<\/a> and <a href=\"https:\/\/www.cdc.gov\/media\/releases\/2020\/s0814-updated-isolation-guidance.html\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/media\/releases\/2020\/s0814-updated-isolation-guidance.html\" target=\"_blank\" rel=\"noreferrer noopener\">\u201cUpdated Isolation Guidance\u201d<\/a>; <a href=\"https:\/\/www.nature.com\/articles\/d41586-020-02400-7\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/d41586-020-02400-7\" target=\"_blank\" rel=\"noreferrer noopener\">Ledford \u201cWhat the immune response\u201d<\/a>).<\/p>\n\n\n\n<p>In contrast, scientists proved that some people were infected a second time with a genetically different strain of SARS-CoV-2.&nbsp; Most of them had more serious symptoms the first time, and less serious or no symptoms the second time.&nbsp; This may show that most people&#8217;s immune systems can defend them from reinfection, and that reinfections might cause mild or no symptoms in some people (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30783-0\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30783-0\/fulltext\" target=\"_blank\">Iwasaki<\/a>; The symptoms most statistically associated with long COVID were fatigue, shortness of breath, memory difficulties, and concentration difficulties (<a href=\"https:\/\/doi.org\/10.1038\/s41598-023-42321-9\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1038\/s41598-023-42321-9\" target=\"_blank\" rel=\"noreferrer noopener\">Marjenberg et al.<\/a>)&nbsp; Some apparently recovered, had no symptoms or viral RNA, and then showed RNA later (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773052\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773052\" target=\"_blank\">Katz<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773053\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773053\" target=\"_blank\">Liotti et al.<\/a>). &nbsp;Some had no symptoms or viral RNA, and then showed symptoms and RNA later. SARS-CoV-2 could attack from tissue reservoirs.&nbsp; Or, SARS-CoV-2 and some host proteins might be partly similar, causing molecular mimicry, so their immune systems could react against their own internal organs, creating autoreactive antibodies (autoantibodies) or autoimmunity.&nbsp; Or, they could have continued cytokine storms, hyperinflammation, microvascular or endothelial damage, mitochondrial dysfunction, hypercoagulation, microbiome changes, or renin-angiotensin system imbalances (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1182\/blood.2021012250\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1182\/blood.2021012250\" target=\"_blank\">Flaumenhaft et al.<\/a>; SARS-CoV-2 and type I interferon could drive inflammation, reduce intestinal absorption of tryptophan, decrease serotonin, hyperactivate platelets, cause thrombocytopenia, impede vagus nerve activity, and impair the hippocampus and memory (<a href=\"https:\/\/doi.org\/10.1016\/j.cell.2023.09.013\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1016\/j.cell.2023.09.013\" target=\"_blank\" rel=\"noreferrer noopener\">Wong AC, Devason, et al.<\/a>).<\/p>\n\n\n\n<p>These varied disease patterns can be classified in three categories: \u201cAcute Infection or COVID-19;\u201d \u201cPostacute Hyperinflammatory Illness;\u201d and \u201cLate Inflammatory and Virological Sequelae\u201d (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2773338\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2773338\" target=\"_blank\">Datta et al.<\/a>).&nbsp; These patterns are similar to other postinfectious syndromes, including post polio syndrome, myalgic encephalomyelitis, chronic fatigue syndrome, fibromyalgia, and post-treatment Lyme disease (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00446-3\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00446-3\" target=\"_blank\">Aucott, Rebman<\/a>; <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/23744235.2021.1924397\" data-type=\"URL\" data-id=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/23744235.2021.1924397\" target=\"_blank\" rel=\"noreferrer noopener\">Yong SJ \u201cLong COVID\u201d<\/a>).  A study in the US found 7.3% had long COVID four weeks after acute COVID-19 (<a href=\"https:\/\/doi.org\/10.1093\/cid\/ciac961\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1093\/cid\/ciac961\" target=\"_blank\" rel=\"noreferrer noopener\">Robertson et al.<\/a>). A study in India found 29.2% had long COVID after four weeks, and 9.4% after six months (<a href=\"https:\/\/doi.org\/10.1371\/journal.pone.0278825\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1371\/journal.pone.0278825\" target=\"_blank\" rel=\"noreferrer noopener\">Arjun et al.<\/a>; <a href=\"https:\/\/cidrap.umn.edu\/covid-19\/estimate-says-19-million-us-adults-have-long-covid\" data-type=\"link\" data-id=\"https:\/\/cidrap.umn.edu\/covid-19\/estimate-says-19-million-us-adults-have-long-covid\" target=\"_blank\" rel=\"noreferrer noopener\">Van Beusekom \u201cEstimate\u201d<\/a>).  More than half did not improve after 1.5 years (<a href=\"https:\/\/doi.org\/10.1016\/j.ijid.2023.10.022\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1016\/j.ijid.2023.10.022\" target=\"_blank\" rel=\"noreferrer noopener\">Agergaard et al.<\/a>; <a href=\"https:\/\/www.cidrap.umn.edu\/covid-19\/study-regardless-variant-half-long-covid-patients-fail-improve-after-18-months\" data-type=\"link\" data-id=\"https:\/\/www.cidrap.umn.edu\/covid-19\/study-regardless-variant-half-long-covid-patients-fail-improve-after-18-months\" target=\"_blank\" rel=\"noreferrer noopener\">Soucheray<\/a>). The numbers of patients accumulated so much that by late 2022, 7% of the US population currently had long COVID, and 7% previously had it (<a href=\"https:\/\/doi.org\/10.1371\/journal.pone.0292672\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1371\/journal.pone.0292672\" target=\"_blank\" rel=\"noreferrer noopener\">Blanchflower et al.<\/a>).<\/p>\n\n\n\n<p>Doing controlled tests on humans to discover how many were infected and developed immunity would be difficult and not-very-ethical (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30982-8\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S1473-3099(20)30982-8\" target=\"_blank\">Spinelli et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/331976\/WHO-2019-nCoV-Ethics_criteria-2020.1-eng.pdf?ua=1\" data-type=\"URL\" data-id=\"https:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/331976\/WHO-2019-nCoV-Ethics_criteria-2020.1-eng.pdf?ua=1\" target=\"_blank\">World Health Organization \u201cKey Criteria\u201d<\/a>), so scientists have been experimenting on animals who get COVID-19 similarly to humans, including hamsters (<a rel=\"noreferrer noopener\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32215622\/\" data-type=\"URL\" data-id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32215622\/\" target=\"_blank\">Chan J, Zhang et al<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.pnas.org\/content\/early\/2020\/06\/19\/2009799117.long\" data-type=\"URL\" data-id=\"https:\/\/www.pnas.org\/content\/early\/2020\/06\/19\/2009799117.long\" target=\"_blank\">Imai et al.<\/a>;&nbsp;<a href=\"https:\/\/doi.org\/10.1080\/22221751.2020.1858177\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1080\/22221751.2020.1858177\" target=\"_blank\" rel=\"noreferrer noopener\">Rosenke et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nature.com\/articles\/s41586-020-2342-5\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41586-020-2342-5\" target=\"_blank\">Sia et al.<\/a>) and rhesus macaques (<a rel=\"noreferrer noopener\" href=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/05\/19\/science.abc4776.long\" data-type=\"URL\" data-id=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/05\/19\/science.abc4776.long\" target=\"_blank\">Chandrashekar et al<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/07\/01\/science.abc5343.long\" data-type=\"URL\" data-id=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/07\/01\/science.abc5343.long\" target=\"_blank\">Deng, Bao et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/ame2.12108\" data-type=\"URL\" data-id=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/ame2.12108\" target=\"_blank\">Yu, Qi et al.<\/a>).&nbsp; They found that some hamsters (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.pnas.org\/content\/early\/2020\/06\/19\/2009799117.long\" data-type=\"URL\" data-id=\"https:\/\/www.pnas.org\/content\/early\/2020\/06\/19\/2009799117.long\" target=\"_blank\">Imai et al.<\/a>) and rhesus macaques (<a rel=\"noreferrer noopener\" href=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/05\/19\/science.abc4776.long\" data-type=\"URL\" data-id=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/05\/19\/science.abc4776.long\" target=\"_blank\">Chandrashekar et al<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/07\/01\/science.abc5343.long\" data-type=\"URL\" data-id=\"https:\/\/science.sciencemag.org\/content\/early\/2020\/07\/01\/science.abc5343.long\" target=\"_blank\">Deng, Bao et al.<\/a>) infected with COVID-19 developed antibodies and were protected against getting infected again.<\/p>\n\n\n\n<p>These different humoral and cellular immunity responses associated with different symptoms of natural COVID-19 infection show why people\u2019s responses to SARS-CoV-2 vaccines vary also (<a href=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" data-type=\"URL\" data-id=\"https:\/\/www.frontiersin.org\/article\/10.3389\/fimmu.2020.571481\" target=\"_blank\" rel=\"noreferrer noopener\">Kim et al.<\/a>).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Biological Herd immunity<\/h2>\n\n\n\n<p>[Publication date of latest article cited: July 1, 2023]<\/p>\n\n\n\n<p>Biological herd immunity occurs when most people develop immunity to the disease pathogen, either from natural infection or vaccination.&nbsp; Some experts hypothetically estimated that perhaps if 43% &#8211; 80% of a population are immune to SARS-CoV-2, then it could not transmit through the whole population (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.mdpi.com\/2076-393X\/8\/2\/236\" data-type=\"URL\" data-id=\"https:\/\/www.mdpi.com\/2076-393X\/8\/2\/236\" target=\"_blank\">Clemente-Su\u00e1rez<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.quantamagazine.org\/the-tricky-math-of-covid-19-herd-immunity-20200630\/\" data-type=\"URL\" data-id=\"https:\/\/www.quantamagazine.org\/the-tricky-math-of-covid-19-herd-immunity-20200630\/\" target=\"_blank\">Hartnett<\/a>; <a href=\"https:\/\/www.cell.com\/immunity\/fulltext\/S1074-7613(20)30170-9\" data-type=\"URL\" data-id=\"https:\/\/www.cell.com\/immunity\/fulltext\/S1074-7613(20)30170-9\" target=\"_blank\" rel=\"noreferrer noopener\">Randolph, Barreiro<\/a>).<\/p>\n\n\n\n<p>But most surveys found much lower percentages have immunity.&nbsp; For example, a survey in Zambia in March \u2013 December 2020, found 10.6% positive for either rt-PCR-detected current infections or ELISA-detected infection history (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00053-X\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00053-X\" target=\"_blank\">Mulenga et al.<\/a>).&nbsp; By December 2020, antibodies increased to 27.6% of blood donors in Nuevo Leon, Mexico (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.3390\/v13071225\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.3390\/v13071225\" target=\"_blank\">Martinez-Acu\u00f1a et al.<\/a>).&nbsp; A study of blood plasma collected from urgent care and routine care patients of a New York City hospital in May &#8211; July 2020 discovered that 20% of the people have antibodies, only a fraction of the number needed for herd immunity (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.nature.com\/articles\/s41586-020-2912-6\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41586-020-2912-6\" target=\"_blank\">Stadlbauer et al.<\/a>). &nbsp;A serological survey of Wuhan found that about 7% had antibodies, after an initial epidemic, then lockdown, and strict control measures (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00238-5\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00238-5\" target=\"_blank\">He Z, Ren, et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00434-7\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00434-7\" target=\"_blank\">Strugnell, Wang<\/a>). &nbsp; A survey of US residents by address found 11.9% had antibodies (<a href=\"https:\/\/doi.org\/10.1093\/cid\/ciab626\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/cid\/ciab626\" target=\"_blank\" rel=\"noreferrer noopener\">Sullivan P, Siegler, et al.<\/a>). A survey of US dialysis patients\u2019 blood samples in July 2020 found about 9% had antibodies, ranging from 3.5% in the west to 27% in the northeast (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32009-2\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32009-2\/fulltext\" target=\"_blank\">Anand et al.<\/a>). &nbsp; A survey of routine care laboratory samples across the US in July &#8211; September 2020 found under 9% had antibodies (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773576\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2773576\" target=\"_blank\">Bajema et al.<\/a>).&nbsp; So, Europeans and Americans had been infected at rates only a fraction of those needed to create herd immunity (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32009-2\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32009-2\/fulltext\" target=\"_blank\">Anand et al.<\/a>).&nbsp; A meta-analysis of 82 seroprevalence surveys world-wide through 2020 found that on the average, 8% of people had antibodies (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00026-7\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00026-7\" target=\"_blank\">Chen X, Chen Z, et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00057-7\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S2214-109X(21)00057-7\" target=\"_blank\">Murhekar, Clapham<\/a>).&nbsp; A review of European COVID-19 statistics in June 2020 found that they were not near herd immunity, and that social distancing and other interventions caused the epidemics to plateau at that time (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)31357-X\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)31357-X\/fulltext\" target=\"_blank\">Okell et al.<\/a>).<\/p>\n\n\n\n<p>But in some populations the percentage with antibodies has come close or exceeded the estimates of the percent needed for herd immunity, and yet more people are still getting infected. &nbsp;This shows that herd immunity probably needs more than those percentages immune.&nbsp; For example, in the USA in May 2021, 83% had infection or vaccine induced antibodies (<a href=\"https:\/\/doi.org\/10.1001\/jama.2021.15161\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jama.2021.15161\" target=\"_blank\" rel=\"noreferrer noopener\">Jones JM, Stone M, et al.<\/a>). In some US prisons 60% &#8211; 80% had been infected (cumulative incidence), and hundreds died (<a href=\"https:\/\/www.themarshallproject.org\/2020\/04\/24\/these-prisons-are-doing-mass-testing-for-covid-19-and-finding-mass-infections\" data-type=\"URL\" data-id=\"https:\/\/www.themarshallproject.org\/2020\/04\/24\/these-prisons-are-doing-mass-testing-for-covid-19-and-finding-mass-infections\" target=\"_blank\" rel=\"noreferrer noopener\">Aspinwall, Neff<\/a>; <a href=\"https:\/\/www.latimes.com\/california\/story\/2020-08-11\/san-quentin-coronavirus-herd-immunity-covid-19\" data-type=\"URL\" data-id=\"https:\/\/www.latimes.com\/california\/story\/2020-08-11\/san-quentin-coronavirus-herd-immunity-covid-19\" target=\"_blank\" rel=\"noreferrer noopener\">Lin, Christensen<\/a>; <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2768249\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2768249\" target=\"_blank\" rel=\"noreferrer noopener\">Saloner et al.<\/a>; <a href=\"https:\/\/doi.org\/10.1089\/jchc.21.03.0024\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1089\/jchc.21.03.0024\" target=\"_blank\" rel=\"noreferrer noopener\">Xu J, Lim A, et al.<\/a>). &nbsp; In Manaus, Brazil by October 2020 an estimated &gt;70% had been infected, but transmission continued (<a href=\"https:\/\/doi.org\/10.1126\/science.abe9728\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1126\/science.abe9728\" target=\"_blank\" rel=\"noreferrer noopener\">Buss et al.<\/a>; <a href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00183-5\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00183-5\" target=\"_blank\" rel=\"noreferrer noopener\">Sabino et al.<\/a>).&nbsp; In an Ecuador village in April 2021, 64% seroconverted while transmission was continuing (<a href=\"https:\/\/doi.org\/10.1177\/21501327211054989\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1177\/21501327211054989\" target=\"_blank\" rel=\"noreferrer noopener\">Del Brutto et al.<\/a>).&nbsp; In Cape Town, South Africa 40% had antibodies, ranging from 31% &#8211; 46% in sub-populations, which may have slowed but not stopped transmission (<a href=\"https:\/\/www.nicd.ac.za\/wp-content\/uploads\/2020\/09\/COVID-19-Special-Public-Health-Surveillance-Bulletin_Issue-5.pdf\" data-type=\"URL\" data-id=\"https:\/\/www.nicd.ac.za\/wp-content\/uploads\/2020\/09\/COVID-19-Special-Public-Health-Surveillance-Bulletin_Issue-5.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Hsiao et al.<\/a>).<\/p>\n\n\n\n<p>These pandemic survey results indirectly predicted the effects of COVID-19 immunization programs.&nbsp; When 8% of a population has been immunized or is naturally immune, probably transmission in the whole population will slow little.&nbsp; When 40% are immunized &nbsp;or naturally immune, transmission will slow moderately.&nbsp; And when 80% are immunized or naturally immune, transmission will slow substantially.&nbsp;&nbsp; Scientists should conduct more random sample population surveys, or samples of pre-natal women, in order to monitor the destructive spread of these viruses, and to predict the constructive progress of immunizations.<\/p>\n\n\n\n<p>Some scientists predicted that societies will probably not reach herd immunity for COVID-19. &nbsp;Even in societies with &gt;80% immune from infections or vaccination, infections are still spreading. &nbsp;Among the remaining people, many refused vaccination. &nbsp;Many do not often access health care systems.&nbsp; As the viruses spread among unvaccinated people, more variant viruses are mutating, and some variants infected people who had infection-induced or vaccine-induced immunity.&nbsp; Mutating variants will continue to infect even some previously infected or vaccinated people.&nbsp; In a race between variants and vaccinations, the variants may continue to stay ahead (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.williamhaseltine.com\/covidvariants\/\" data-type=\"URL\" data-id=\"https:\/\/www.williamhaseltine.com\/covidvariants\/\" target=\"_blank\">Haseltine<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1093\/infdis\/jiac109\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/infdis\/jiac109\" target=\"_blank\">Morens et al.<\/a>).&nbsp; Among vaccinated and naturally infected people, many people\u2019s immunity wanes (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1038\/d41586-021-00728-2\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1038\/d41586-021-00728-2\" target=\"_blank\">Aschwanden<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.nytimes.com\/2021\/05\/03\/health\/covid-herd-immunity-vaccine.html\" data-type=\"URL\" data-id=\"https:\/\/www.nytimes.com\/2021\/05\/03\/health\/covid-herd-immunity-vaccine.html\" target=\"_blank\">Mandavilli \u201cReaching \u2018Herd Immunity\u2019 Is Unlikely\u201c<\/a>) and only partially protects them from infection.  For example, one review found that infection resulted in only 24.7% protective effectiveness against reinfection at 12 months (<a href=\"https:\/\/doi.org\/10.1016\/S1473-3099(22)00801-5\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1016\/S1473-3099(22)00801-5\" target=\"_blank\" rel=\"noreferrer noopener\">Bobrovitz et al.<\/a>). A meta-analysis found that 12 months after infection, 17% had IgM antibodies and 75% had IgG antibodies, showing that many were probably not immune (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1186\/s12879-023-08425-3\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1186\/s12879-023-08425-3\" target=\"_blank\">Li Q, Chen, et al.<\/a>).  So, we should call this situation \u201cherd resistance\u201d (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1080\/17425247.2023.2189697\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1080\/17425247.2023.2189697\" target=\"_blank\">Hussain et al.<\/a>).&nbsp;In this situation, using many prevention methods can mitigate but not eliminate transmission (<a href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00978-8\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00978-8\">Oliu-Barton et al.<\/a>).<\/p>\n\n\n\n<p>Some scientists advocated allowing healthy members of the public to live and work as they did before the pandemic, get infected, and develop natural herd immunity, while protecting more vulnerable people from infection (<a href=\"https:\/\/gbdeclaration.org\/\" data-type=\"URL\" data-id=\"https:\/\/gbdeclaration.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">Kulldorff et al.<\/a>)&nbsp; Others disagreed, saying this would cause much suffering; it is not feasible to identify, isolate, and protect large numbers of vulnerable people; and many public health measures would better protect people (<a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32153-X\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)32153-X\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Alwan, Burgess, et al.<\/a>; This debate will continue (<a href=\"https:\/\/www.thelancet.com\/journals\/lanres\/article\/PIIS2213-2600(20)30555-5\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.thelancet.com\/journals\/lanres\/article\/PIIS2213-2600(20)30555-5\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Burki \u201cHerd Immunity\u201d<\/a>).<\/p>\n\n\n\n<p>This concept of natural herd immunity for COVID-19 seems to be based on four inaccurate assumptions about pathology, psychology, and immunity.<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>It assumes that SARS-CoV-2 causes four conditions: symptomatic infections, asymptomatic infections, immunity, or death.&nbsp; But it can also cause long-term symptoms (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2768351\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2768351\" target=\"_blank\">Carfi et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamacardiology\/fullarticle\/2768915\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamacardiology\/fullarticle\/2768915\" target=\"_blank\">Yancy, Fonarow<\/a>).<\/li>\n\n\n\n<li>If governments relaxed some restrictions that would relieve some economic problems and anxieties.&nbsp; But people would notice others\u2019 short and long term symptoms and organ damages, fear the consequences, and impose restrictions on each other.&nbsp; Many families, organizations, villages, etc. would debate and adopt widely varying rules, some science-based, others imagination-based, some stricter than government rules.&nbsp; Some of these would cause more economic problems and anxieties than the recent government restrictions.<\/li>\n\n\n\n<li>It assumes that infection results in immunity or death.&nbsp; But follow-up of Danish people found that natural infection had only 80% protective effect against later infection among people under 65, and 47% among those 65 and older. This implies that 20% of the younger people did not develop immunity, and 53% of the older people (<a href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00662-0\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00662-0\" target=\"_blank\" rel=\"noreferrer noopener\">Boyton, Altmann<\/a>; <a href=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00575\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S0140-6736(21)00575\" target=\"_blank\" rel=\"noreferrer noopener\">Hansen, et al.<\/a>).<\/li>\n\n\n\n<li>When large numbers of people have COVID-19, medical workers and organizations are overstressed, preventing them from providing the best care. &nbsp;So, more COVID-19 patients die than when smaller numbers of patients have COVID-19.&nbsp; More patients with other diseases get severely ill and die (<a href=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" data-type=\"URL\" data-id=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" target=\"_blank\" rel=\"noreferrer noopener\">Christakis<\/a>; <a href=\"https:\/\/global.oup.com\/academic\/product\/covids-impact-on-health-and-healthcare-workers-9780197575390\" data-type=\"URL\" data-id=\"https:\/\/global.oup.com\/academic\/product\/covids-impact-on-health-and-healthcare-workers-9780197575390\" target=\"_blank\" rel=\"noreferrer noopener\">Goldenberg<\/a>).<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Social Herd Immunity<\/strong><\/h2>\n\n\n\n<p>[Publication date of latest article cited: March 2023]<\/p>\n\n\n\n<p>Social herd immunity for an infectious disease occurs when most people know someone who got sickened with that disease, and talk to each other about effective prevention methods.&nbsp; When COVID-19 started spreading through big cities, those residents knew people who then got the disease, so people started telling others about it.&nbsp; In towns and rural areas, large numbers of people did not yet know anyone diseased, so some denied the seriousness of this disease or, resisted using scientifically valid COVID-19 prevention methods.&nbsp; As more were sickened, more started talking realistically about it, the process by which communities develop social herd immunity to a disease.<\/p>\n\n\n\n<p>This affected people\u2019s perceptions of the pandemic. &nbsp; A survey in February \u2013 March 2021, after a year of the pandemic, found that 19% of Americans know a close friend or relative who died of COVID-19.&nbsp; 67% worried that they or a household member will get COVID-19.&nbsp; 65% always wear a mask outside home near other people (<a href=\"https:\/\/apnorc.org\/projects\/the-coronavirus-pandemic-one-year-later\/\" data-type=\"URL\" data-id=\"https:\/\/apnorc.org\/projects\/the-coronavirus-pandemic-one-year-later\/\" target=\"_blank\" rel=\"noreferrer noopener\">AP-NORC Center<\/a>). &nbsp;Among New York City residents \u2265 70 years old, 63% knew someone diagnosed with COVID-19, and 51% knew someone who died of it, by March 2021.&nbsp; 54% thought about themselves or another person dying of COVID-19 (<a href=\"https:\/\/doi.org\/10.1007\/s10900-021-01061-5\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1007\/s10900-021-01061-5\" target=\"_blank\" rel=\"noreferrer noopener\">Greenleaf et al.<\/a>). &nbsp;These studies show that large portions of Americans are attaining social herd immunity.&nbsp; And yet, by mid-2020, many people felt tired of prevention methods and said the pandemic was ending when infections were actually increasing (<a href=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" data-type=\"URL\" data-id=\"https:\/\/www.littlebrown.com\/titles\/nicholas-a-christakis-md-phd\/apollos-arrow\/9780316628204\/\" target=\"_blank\" rel=\"noreferrer noopener\">Christakis<\/a>).<\/p>\n\n\n\n<p>Some authors used \u201csocial herd immunity\u201d with other informative meanings.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sun used \u201csocial herd immunity\u201d in a way that meant biological herd immunity in society (<a href=\"https:\/\/doi.org\/10.1016\/j.asoc.2023.110081\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.asoc.2023.110081\" target=\"_blank\" rel=\"noreferrer noopener\">Sun L, He Q, et al.<\/a>).<\/li>\n\n\n\n<li><a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2766096\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2766096\" target=\"_blank\">Williams and Cooper<\/a> wrote that \u201cThe US must develop a new kind of \u2018herd immunity,\u2019 whereby resistance to the spread of poor health in the population occurs when a sufficiently high proportion of individuals, across all racial, ethnic, and social class groups, are protected from and thus \u2018immune\u2019 to negative social determinants.\u201d This meaning is broader than the one above, because it includes many actions and ideas preventing many diseases. Disparities in this kind of social herd immunity did affect COVID-19 transmission.&nbsp; For example, in Washington DC, neighborhoods with higher concentrations of African Americans and low income people, there were more COVID-19 cases, and less testing than other neighborhoods (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.annepidem.2021.04.003\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.annepidem.2021.04.003\" target=\"_blank\">Brown K, Lewis, et al.<\/a>). &nbsp;In California, Latinx people are more likely to live in high-exposure-risk households, are overrepresented in cumulative cases, and are underrepresented in cumulative testing. &nbsp;Blacks have a higher death rate, and Asians have a lower testing rate (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1377\/hlthaff.2021.00098\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1377\/hlthaff.2021.00098\" target=\"_blank\">Reitsma et al.<\/a>).<\/li>\n\n\n\n<li><a href=\"https:\/\/theappeal.org\/coronavirus-racial-disparities-residential-segregation\/\" data-type=\"URL\" data-id=\"https:\/\/theappeal.org\/coronavirus-racial-disparities-residential-segregation\/\" target=\"_blank\" rel=\"noreferrer noopener\">Wise<\/a>, interviewing Alang, gave examples of that broad meaning, such as more upper income people working with information through the internet from home safe from COVID-19 transmission, while only small percentages of low income people can work from home. This reduces transmission in upper income neighborhoods: \u201cRegardless of individual income, all residents of these privileged neighborhoods benefit from a kind of \u201csocial herd immunity\u201d against the virus, while all residents of disadvantaged neighborhoods are at greater risk. Alang posed the scenario: If you shop at a grocery store where everyone is exposed because there\u2019s an essential worker in every single house, that increases your exposure. &nbsp;But if you work in a neighborhood where everyone can work from home, then that reduces your exposure, she said.\u201d<\/li>\n\n\n\n<li><a rel=\"noreferrer noopener\" href=\"https:\/\/www.sciencemediacentre.org\/expert-comments-about-herd-immunity\/\" data-type=\"URL\" data-id=\"https:\/\/www.sciencemediacentre.org\/expert-comments-about-herd-immunity\/\" target=\"_blank\">van Schaik<\/a> described mutual aid: \u201cCommunities will need to come together to provide \u2018social herd immunity\u2019 by helping those out that are in quarantine (for example, by checking in on them via social media or direct messaging apps and doing some shopping for them.\u201d<\/li>\n\n\n\n<li>Scata explained a similar idea using systems analysis, to protect and isolate some infection-susceptible, disadvantaged network nodes (<a href=\"https:\/\/doi.org\/10.1038\/srep37105\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1038\/srep37105\" target=\"_blank\" rel=\"noreferrer noopener\">Scata et al.<\/a>).<\/li>\n\n\n\n<li><a href=\"https:\/\/www.theverge.com\/2019\/3\/5\/18251807\/senate-anti-vax-vaccines-congressional-hearing-ethan-lindenberger-ohio-teen\" data-type=\"URL\" data-id=\"https:\/\/www.theverge.com\/2019\/3\/5\/18251807\/senate-anti-vax-vaccines-congressional-hearing-ethan-lindenberger-ohio-teen\" target=\"_blank\" rel=\"noreferrer noopener\">Robertson<\/a> used social herd immunity to mean society\u2019s understanding and support for vaccines persuades more people to get vaccinated, \u201cthat protects people who can\u2019t be vaccinated for health reasons\u201d.<\/li>\n\n\n\n<li>Some authors used \u201cherd immunity\u201d as a virology metaphor for other topics. <a href=\"https:\/\/doi.org\/10.1111\/ina.12970\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1111\/ina.12970\" target=\"_blank\" rel=\"noreferrer noopener\">Nazaroff<\/a>). &nbsp;But doing those activities outdoors, or keeping quiet or speaking softly indoors (such as theaters, classrooms, or offices), did not spread COVID-19 to large numbers (<a href=\"https:\/\/quillette.com\/2020\/04\/23\/covid-19-superspreader-events-in-28-countries-critical-patterns-and-lessons\/\" data-type=\"URL\" data-id=\"https:\/\/quillette.com\/2020\/04\/23\/covid-19-superspreader-events-in-28-countries-critical-patterns-and-lessons\/\" target=\"_blank\" rel=\"noreferrer noopener\">Kay<\/a>; <a href=\"https:\/\/doi.org\/10.1126\/science.abd9149\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1126\/science.abd9149\" target=\"_blank\" rel=\"noreferrer noopener\">Wang C, Prather K, et al.<\/a>).&nbsp; Superspreader events have been classified as \u201cSocietal\u201d (in which all members could potentially transmit to outsiders, and thus are difficult to control) and \u201cisolated\u201d (few members could transmit outside, and could be quarantined) (<a href=\"https:\/\/www.journalofinfection.com\/article\/S0163-4453(20)30717-9\/fulltext\" data-type=\"URL\" data-id=\"https:\/\/www.journalofinfection.com\/article\/S0163-4453(20)30717-9\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Majra et al.<\/a>).&nbsp; Small numbers of people transmitting in superspreading or cluster outbreaks probably cause a disproportionate amount of transmission, called \u201coverdispersion in transmission\u201d (<a href=\"https:\/\/science.sciencemag.org\/content\/370\/6515\/406\" data-type=\"URL\" data-id=\"https:\/\/science.sciencemag.org\/content\/370\/6515\/406\" target=\"_blank\" rel=\"noreferrer noopener\">Lee E, Wada et al.<\/a>)<\/p>\n\n\n\n<p>Generally, outdoor venues are safer, indoor places with flow-through ventilation are less safe, and indoor places with little ventilation are the least safe (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.theatlantic.com\/politics\/archive\/2020\/11\/can-you-get-coronavirus-inside-restaurant\/617151\/\" data-type=\"URL\" data-id=\"https:\/\/www.theatlantic.com\/politics\/archive\/2020\/11\/can-you-get-coronavirus-inside-restaurant\/617151\/\" target=\"_blank\">Khazan<\/a>; During the pandemic phase, studies of cell phone mobility comparing US counties and census block groups found that more travel to less-essential businesses (restaurants, fitness centers, and hotels) was associated with higher transmission (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.nature.com\/articles\/s41586-020-2923-3\" data-type=\"URL\" data-id=\"https:\/\/www.nature.com\/articles\/s41586-020-2923-3\" target=\"_blank\">Chang S, Pierson et al.<\/a>; More staying in residences was associated with less increases in COVID-19 incidence (<a rel=\"noreferrer noopener\" href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769771\" data-type=\"URL\" data-id=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2769771\" target=\"_blank\">Sehra et al.<\/a>).&nbsp; A study comparing COVID-19 outpatients to other matched control-participants found that eating in restaurants was the variable most associated with infection (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6936a5.htm\" data-type=\"URL\" data-id=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6936a5.htm\" target=\"_blank\">Fisher KA, Tenforde, et al.<\/a>). &nbsp;US Counties allowing on-premises dining in restaurants had COVID-19 case growth rates 40 \u2013 100 days after reopening (<a rel=\"noreferrer noopener\" href=\"http:\/\/dx.doi.org\/10.15585\/mmwr.mm7010e3\" data-type=\"URL\" data-id=\"http:\/\/dx.doi.org\/10.15585\/mmwr.mm7010e3\" target=\"_blank\">Guy et al.<\/a>).&nbsp; In order to help those businesses continue financially through the pandemic phase, and gain their benefits, many people are buying their products and services safely.&nbsp; For example, they ate restaurant and bar food and drinks outdoors, or took it home, or exercised outside gyms, or paid for cooking, nutrition, or exercise lessons from restaurant, bar, and gym personnel.<\/p>\n\n\n\n<p>During winter, more people congregate indoors.&nbsp; Cold winter air is often lower humidity than warm summer air, and heating it further reduces humidity, enabling SARS-CoV-2 laden droplets to dry, and float longer.&nbsp; This could increase COVID-19 transmission. &nbsp;(<a href=\"https:\/\/aaqr.org\/articles\/aaqr-20-06-covid-0302\" data-type=\"URL\" data-id=\"https:\/\/aaqr.org\/articles\/aaqr-20-06-covid-0302\" target=\"_blank\" rel=\"noreferrer noopener\">Ahlawat et al.<\/a>; <a href=\"https:\/\/www.washingtonpost.com\/opinions\/2020\/11\/18\/winter-covid-19-humidity\/\" data-type=\"URL\" data-id=\"https:\/\/www.washingtonpost.com\/opinions\/2020\/11\/18\/winter-covid-19-humidity\/\" target=\"_blank\" rel=\"noreferrer noopener\">Allen et al.<\/a>; The lipid envelopes of enveloped viruses (such as influenza and probably SARS-CoV-2) solidify at low temperatures, and can last longer before being inactivated.&nbsp; Colder temperatures and lower humidity impair the cilia (hairlike covering) and mucous clearing foreign particles from people&#8217;s airways (<a href=\"https:\/\/www.medpagetoday.com\/infectiousdisease\/covid19\/89495\" data-type=\"URL\" data-id=\"https:\/\/www.medpagetoday.com\/infectiousdisease\/covid19\/89495\" target=\"_blank\" rel=\"noreferrer noopener\">Hassad<\/a>; <a href=\"https:\/\/www.annualreviews.org\/doi\/10.1146\/annurev-virology-012420-022445\" data-type=\"URL\" data-id=\"https:\/\/www.annualreviews.org\/doi\/10.1146\/annurev-virology-012420-022445\" target=\"_blank\" rel=\"noreferrer noopener\">Moriyama et al.<\/a>) and antiviral defense and tissue repair (<a href=\"https:\/\/www.washingtonpost.com\/opinions\/2020\/11\/18\/winter-covid-19-humidity\/\" data-type=\"URL\" data-id=\"https:\/\/www.washingtonpost.com\/opinions\/2020\/11\/18\/winter-covid-19-humidity\/\" target=\"_blank\" rel=\"noreferrer noopener\">Allen et al.<\/a>; This occurred perhaps because of decreased testing or increased COVID-19 prevention. (<a href=\"https:\/\/doi.org\/10.1016\/S2213-2600(20)30508-7\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/S2213-2600(20)30508-7\" target=\"_blank\" rel=\"noreferrer noopener\">Burki \u201cDouble Threat\u201d<\/a>).<\/p>\n\n\n\n<p>To reduce indoor transmission, many people have been opening windows and pumping more outside air into buildings (<a rel=\"noreferrer noopener\" href=\"https:\/\/academic.oup.com\/cid\/advance-article\/doi\/10.1093\/cid\/ciaa1270\/5898577\" data-type=\"URL\" data-id=\"https:\/\/academic.oup.com\/cid\/advance-article\/doi\/10.1093\/cid\/ciaa1270\/5898577\" target=\"_blank\">de Man et al.;<\/a> <a rel=\"noreferrer noopener\" href=\"https:\/\/www.canoncitydailyrecord.com\/2020\/08\/07\/cu-boulder-ventilation-coronavirus\/\" data-type=\"URL\" data-id=\"https:\/\/www.canoncitydailyrecord.com\/2020\/08\/07\/cu-boulder-ventilation-coronavirus\/\" target=\"_blank\">Hernandez<\/a>; Others are attempting to reduce these problems by setting up tents or plastic bubbles outside restaurants in which air flows more than indoors, and by having customers wear masks when not eating, staff wear masks and clean often, and maintain social distancing (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.ashrae.org\/file%20library\/technical%20resources\/covid-19\/guidance-for-small-temporary-dining-structures.pdf\" data-type=\"URL\" data-id=\"https:\/\/www.ashrae.org\/file%20library\/technical%20resources\/covid-19\/guidance-for-small-temporary-dining-structures.pdf\" target=\"_blank\">ASHRAE \u201cGuidance for Small Temporary Dining Structures\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.safety.com\/coronavirus\/restaurant-safety-survey\/\" data-type=\"URL\" data-id=\"https:\/\/www.safety.com\/coronavirus\/restaurant-safety-survey\/\" target=\"_blank\">Starr<\/a>).<\/p>\n\n\n\n<p>To help people understand and prevent airborne transmission risks, experts developed an online spreadsheet in which one can enter information on the characteristics of a place, and the spreadsheet will estimate numbers of people who could get infected (<a href=\"https:\/\/tinyurl.com\/covid-estimator\" data-type=\"URL\" data-id=\"https:\/\/tinyurl.com\/covid-estimator\" target=\"_blank\" rel=\"noreferrer noopener\">Jimenez<\/a>).&nbsp; Another model estimates safe limits on \u201ccumulative exposure time\u201d indoors, depending on the number people, time, ventilation, room dimensions, breathing rate, respiratory activity, face-mask use, and respiratory aerosols infectiousness (<a href=\"http:\/\/web.mit.edu\/bazant\/www\/COVID-19\/\" data-type=\"URL\" data-id=\"http:\/\/web.mit.edu\/bazant\/www\/COVID-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">Bazant<\/a>; <a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.08.26.20182824v1\" data-type=\"URL\" data-id=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.08.26.20182824v1\" target=\"_blank\" rel=\"noreferrer noopener\">Bazant, Bush<\/a>).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Airplanes, buses, and cars<\/h2>\n\n\n\n<p>[Publication date of latest article cited: October 26, 2023]<\/p>\n\n\n\n<p>It would be not-very-ethical to experiment on whether humans can get COVID-19 infection from aerosols in enclosed spaces, such as airplanes and buses (<a rel=\"noreferrer noopener\" href=\"https:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/331976\/WHO-2019-nCoV-Ethics_criteria-2020.1-eng.pdf?ua=1\" data-type=\"URL\" data-id=\"https:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/331976\/WHO-2019-nCoV-Ethics_criteria-2020.1-eng.pdf?ua=1\" target=\"_blank\">World Health Organization \u201cKey Criteria\u201d<\/a>).&nbsp; But people created several natural experiments that scientists studied.&nbsp; The Korean Centers for Disease Control studied whether people got infected on air flights evacuating Koreans from Italy during the epidemic there.&nbsp; They found that two probably previously uninfected passengers were infected during the flights.&nbsp; But it is uncertain if they received the SARS-CoV-2 viruses from fomites or aerosols (<a rel=\"noreferrer noopener\" href=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/26\/11\/20-3353_article\" data-type=\"URL\" data-id=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/26\/11\/20-3353_article\" target=\"_blank\">Bae, Shin, et al<\/a>).&nbsp; These natural experiments show that aerosols or fomite surfaces can transmit these infections during air flights.<\/p>\n\n\n\n<p>People transmitted on buses also.&nbsp; Early in the pandemic, hundreds of people traveled to an outdoor Buddhist ceremony in China.&nbsp; One person had just started symptoms, and traveled on a bus to and from the ceremony.&nbsp; The only people subsequently infected either rode the same enclosed bus with recycling air conditioning, or were close to the infected person at the ceremony (<a href=\"https:\/\/www.nytimes.com\/2020\/09\/01\/health\/coronavirus-bus-china.html\" data-type=\"URL\" data-id=\"https:\/\/www.nytimes.com\/2020\/09\/01\/health\/coronavirus-bus-china.html\" target=\"_blank\" rel=\"noreferrer noopener\">Rabin; Shen et al.<\/a>).&nbsp; One infected person probably transmitted to several others on a bus and a minivan, mainly to people who were not wearing masks and were downwind of the internal air flow (<a href=\"https:\/\/doi.org\/10.1093\/ofid\/ofaa430\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/ofid\/ofaa430\" target=\"_blank\" rel=\"noreferrer noopener\">Luo et al.<\/a>).<\/p>\n\n\n\n<p>Reviews of published, peer-reviewed reports of COVID-19 risk and transmission on airplanes showed many cases of secondary transmission occurred on flights (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1111\/irv.12846\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1111\/irv.12846\" target=\"_blank\">Blomquist et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1093\/jtm\/taab133\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/jtm\/taab133\" target=\"_blank\">Rosca et al.<\/a>).&nbsp; Transmission occurred when few people wore masks, and few or no transmission cases occurred when almost all people wore masks (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1093\/jtm\/taaa117\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/jtm\/taaa117\" target=\"_blank\">Nir-Paz et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/academic.oup.com\/jtm\/advance-article\/doi\/10.1093\/jtm\/taaa178\/5910636\" data-type=\"URL\" data-id=\"https:\/\/academic.oup.com\/jtm\/advance-article\/doi\/10.1093\/jtm\/taaa178\/5910636\" target=\"_blank\">Freedman, Wilder-Smith<\/a>).&nbsp; Transmission probably occurred to several people during long waits in international airports and a flight to Ireland, when some infected people and some uninfected people wore masks and some did not.&nbsp; The infected people then spread COVID-19 to others across Ireland (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.washingtonpost.com\/travel\/2020\/10\/26\/ireland-flight-covid\/\" data-type=\"URL\" data-id=\"https:\/\/www.washingtonpost.com\/travel\/2020\/10\/26\/ireland-flight-covid\/\" target=\"_blank\">McMahon<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.2807\/1560-7917.ES.2020.25.42.2001624\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.2807\/1560-7917.ES.2020.25.42.2001624\" target=\"_blank\">Murphy et al<\/a>). &nbsp;In May \u2013 September 2020, 2,866 infected people traveled on airplanes, and transmitted to 44 people, among 1.4 billion passengers.&nbsp; So, the risk of transmission is about one in 1.7 million (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1016\/j.tmaid.2021.102133\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1016\/j.tmaid.2021.102133\" target=\"_blank\">Pang et al.<\/a>). The attack rates from infected passengers to infecting others were 5.1% without mandatory masking and 3.6% with mandatory masking (<a href=\"https:\/\/doi.org\/10.1155\/2022\/8364666\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1155\/2022\/8364666\" target=\"_blank\" rel=\"noreferrer noopener\">Moek, et al.<\/a>).<\/p>\n\n\n\n<p>Scientists also experimented with measuring floating aerosol particles, and falling particles deposited on surfaces in commercial passenger airplanes.&nbsp; They found low densities of particles in the air, probably \u201c\u2026due to both airframes\u2019 high air exchange rates, downward ventilation design, and HEPA-filtered recirculation.\u201c&nbsp; They found particles on horizontal surfaces, but less on vertical surfaces (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1111\/ina.12974\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1111\/ina.12974\" target=\"_blank\">Hanna<\/a>; <a href=\"https:\/\/thehill.com\/policy\/defense\/521303-pentagon-study-low-risk-of-contracting-covid-19-on-planes-with-passengers\" data-type=\"URL\" data-id=\"https:\/\/thehill.com\/policy\/defense\/521303-pentagon-study-low-risk-of-contracting-covid-19-on-planes-with-passengers\" target=\"_blank\" rel=\"noreferrer noopener\">Mitchell<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.ustranscom.mil\/cmd\/docs\/TRANSCOM%20Report%20Final.pdf\" data-type=\"URL\" data-id=\"https:\/\/www.ustranscom.mil\/cmd\/docs\/TRANSCOM%20Report%20Final.pdf\" target=\"_blank\">Silcott et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.ustranscom.mil\/cmd\/panewsreader.cfm?ID=C0EC1D60-CB57-C6ED-90DEDA305CE7459D\" data-type=\"URL\" data-id=\"https:\/\/www.ustranscom.mil\/cmd\/panewsreader.cfm?ID=C0EC1D60-CB57-C6ED-90DEDA305CE7459D\" target=\"_blank\">Wade<\/a>).&nbsp; For these reasons, there may be less risk of getting COVID-19 in a passenger plane than in most buildings (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1001\/jama.2020.19108\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1001\/jama.2020.19108\" target=\"_blank\">Pombal, et al.<\/a>).<\/p>\n\n\n\n<p>Airlines and health experts experimented with methods of decreasing transmission in airplanes and airports.&nbsp; For example, during the pandemic many airlines and airports required mask wearing at all times except when eating, which employees enforce (<a rel=\"noreferrer noopener\" href=\"https:\/\/npli.sph.harvard.edu\/crisis-research\/aviation-public-health-initiative-aphi\/\" data-type=\"URL\" data-id=\"https:\/\/npli.sph.harvard.edu\/crisis-research\/aviation-public-health-initiative-aphi\/\" target=\"_blank\">Harvard \u201cAviation Public Health Initiative\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/cdn1.sph.harvard.edu\/wp-content\/uploads\/sites\/2443\/2020\/10\/HSPH-APHI-Phase-One-Report.pdf\" data-type=\"URL\" data-id=\"https:\/\/cdn1.sph.harvard.edu\/wp-content\/uploads\/sites\/2443\/2020\/10\/HSPH-APHI-Phase-One-Report.pdf\" target=\"_blank\">Harvard \u201cAssessment of Risks\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/www.united.com\/ual\/en\/us\/fly\/travel\/what-to-expect.html\" data-type=\"URL\" data-id=\"https:\/\/www.united.com\/ual\/en\/us\/fly\/travel\/what-to-expect.html\" target=\"_blank\">United<\/a>). &nbsp;The US Centers for Disease Control and Prevention tried screening people\u2019s symptoms and temperatures at airports, but found this ineffective for the amount of work finding small numbers of cases (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.cnn.com\/travel\/article\/cdc-study-symptom-screening-airport-covid-19-ineffective\/index.html\" data-type=\"URL\" data-id=\"https:\/\/www.cnn.com\/travel\/article\/cdc-study-symptom-screening-airport-covid-19-ineffective\/index.html\" target=\"_blank\">Christensen<\/a>; <a rel=\"noreferrer noopener\" href=\"http:\/\/dx.doi.org\/10.15585\/mmwr.mm6945a4\" data-type=\"URL\" data-id=\"http:\/\/dx.doi.org\/10.15585\/mmwr.mm6945a4\" target=\"_blank\">Dollard et al.<\/a>).  Airports could increase monitoring of SARS-CoV-2 in the air (<a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1039\/d3cs00417a\" data-type=\"link\" data-id=\"https:\/\/doi.org\/10.1039\/d3cs00417a\" target=\"_blank\">Qiu G, Zhang X, et al.<\/a>).<\/p>\n\n\n\n<p>COVID-19 risks on airplanes, trains, buses, and taxis vary according to the air circulation, air filters, seat locations, surface cleaning, and trip time (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.scientificamerican.com\/article\/evaluating-covid-risk-on-planes-trains-and-automobiles2\/\" data-type=\"URL\" data-id=\"https:\/\/www.scientificamerican.com\/article\/evaluating-covid-risk-on-planes-trains-and-automobiles2\/\" target=\"_blank\">Bushwick et al.<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1093\/trstmh\/traa106\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1093\/trstmh\/traa106\" target=\"_blank\">Harries, Martinez, et al.<\/a>).&nbsp; In a computer aided design (CAD) of air flows in a moving car, having all the windows open created the least risk of aerosols flowing from an infected person to an uninfected person, two or three windows open on opposite sides created less safe airflows, and all windows closed created the most risky airflows (<a rel=\"noreferrer noopener\" href=\"https:\/\/www.medicalnewstoday.com\/articles\/study-models-airflow-inside-a-car-to-help-reduce-sars-cov-2-transmission\" data-type=\"URL\" data-id=\"https:\/\/www.medicalnewstoday.com\/articles\/study-models-airflow-inside-a-car-to-help-reduce-sars-cov-2-transmission\" target=\"_blank\">Huzar, Flynn \u201cStudy Models Airflow\u201d<\/a>; <a rel=\"noreferrer noopener\" href=\"https:\/\/doi.org\/10.1126\/sciadv.abe0166\" data-type=\"URL\" data-id=\"https:\/\/doi.org\/10.1126\/sciadv.abe0166\" target=\"_blank\">Mathai et al.<\/a>).<\/p>\n\n\n\n<p>Even after the pandemic had changed to a more endemic phase in 2022, it is still potentially possible to get exposed to SARS-CoV-2 on air flights.
